Mary, I've never had this problem with either of my phones, until I upgraded to this 16.2. I regret ever upgrading to 16.2! I've never had so many problems with either of my phones until I upgraded to 16.2. I have a very serious problem with my Apple Play lists as well, I disconnected all my Bluetooth devices from the phone, and had to reconnect them again. I can't even get Siri to set my alarm clock. This is nuts! I upgraded to 16.2, and I had to erase my entire phone, I had to rename the phone, I lost all my phone calls in my recent folder in the phone app. I don't know what else to do. I was seriously considering buying the new iPhone 14, but I quickly changed my mine. I can't raise or lower the volume on the phone, so when I am talking on the phone, people keep saying they can't hear me, and there isn't anything I can do about that either!

It seems to me that because I can hear it sometimes and not other times, there might be something going on with audio ducking. It seems to me like voice over having been speaking, just as I pressed the side button to invoke Siri is somehow responsible for not hearing the tone.
