Well, on my 13 pro, using BARDMobile with Screen Recognition is awful. In places it says Image, a white letter on a gray background … or something equally useless.

If you're noticing an app that used to work and now VoiceOver doesn't seem to be speaking much of anything, try using the rotor to turn on screen recognition for that app to see if you get different results. I seldom leave it turned on for an app when doing this but I expect you'll develop a routine that works best for you.
