Mark, you were half way there. You did a double tap and hold which is what you should do. The second part of that is lift up your fingers up and just double tap the app or apps you want to remove from the switcher.

So, what's the step I'm missing here? Conversely, sometimes, if I launch an app from the home screen, it does *NOT* appear in App Switcher, even though I want it to. Thus the subject line.
