Hi Kerri, Most apps have a link to either an email or support site. This could be in Help, About or More, depending how the app is laid out. You can, of course, always Google an app, find the developer's website and see if there is a contact form or email address listed.
As for what to say, well, I usually tell the developer that I am a blind iPhone user and have to rely on Voiceover and Apple's fantastic built-in accessibility features to use my iPhone. I also often say that i am part of a google group where over 1,700 visually impaired iPhone users help each other and discuss apps and accessibility and that this list is constantly growing because Apple offers great accessibility and also more and more developers follow apple's accessibility guidelines to make sure their apps can be used by a Voiceover user.
