Ahh....You're in Settings -> Control Center. The only way to access the actual Control Center to my knowledge is the 3 finger swipe up from the Home screen or Lock screen, which I know you said was difficult and can understand your frustration. I would suggest writing to Apple Accessibility to explain the difficulty. It seems there should definitely be another option to get to the orientation lock on the iPhone.
