Not sure about the 6096 but on the TNT the modem gets flagged suspect if it
has 3 failed connections within a time frame and if it continues the modem
will be disabled. On the TNT you can view the suspect modems by using the
modem -s command. You might want to give it a try.

I suspect that I have a bad modem in a Max 6096. Users are complaining of
hitting a dead modem on our end. Call makes connection, but immediately
disconnects or MAX does not return connection tones.

Does anybody have a way to isolate the problem card?

>From what I have been able to find out, it appears that the MAX assigns
calls randomly to modems. Is this correct? If it is, what is the best way to
determine the defective card?
