So I ran Disk Warrior 4.1.1 over my 10.5.6
The first run was with Disk Warrior installed on a Hard drive and
using a 10.5.6 system
on another volume. All ran well and Disk Warrior completed it's tasks .
So then I realised you may have run from DVD , so ran it from DVD over
the previous operating system also 10.5.6
Again it completed it's tasks .
NOTE : From DVD it boots with 10.4.8 ..... so what was your iMac
original
operating system ?

What Version of DiskWarrior are you using, are you using the
latest version for your OS? You don't say if you are using
Leopard.
DW 4.1.1 is latest version.
It's possible that you have a bad spot (bad sector) on the hard
drive. DW only fixes the directory. It does not fix other disk-
related problems that may be repaired by Disk Utility or
TechTool Pro.
Have you checked the SMART status of your drive with DiskWarrior?
