When viewing an app's instances in JavaMonitor, does JavaMonitor report not being able to communicate with wotaskd?

No. It just reports after waiting for sometime after the logs show that it's waiting for requests ... death.

If not using Mac OS X Server 10.5 and WO 5.4, have you inserted the following:

FreeBSD.

Interestingly this has only just started happening for me today. I can install the app onto my local system and launch it with JM without problems. Weird.
