William, I agree that we need to take reasonable and appropriate steps
to protect PHI, I believe it strongly. But, when you get to actual "care
givers"....there is a saturation point where, when asked to take a "few
more steps" to make sure an email is secure,  you hit a wall of
resistance that is very difficult to overcome. There has to be a very
good balance between ease of use on both the sending and receiving side,
and how "tight and secure" the system is. 

Some of the systems require a lot of setup up front...and that can
increase your IT burden. Some of them require a pretty sophisticated
enduser, and could turn-off patients. I think the technology has a way
to go before the majority of email users are comfortable with it.
Regardless, I believe most providers are under a burden to
encrypt....and will have to deal with the consequences. My opinion only.
