Well, people using Windows and MacOS X don't care about using a 100%
FOSS VoIP solution otherwise they wouldn't be using their OS in the
first place. They can always interface with FOSS clients using the
myriad of proprietary SIP clients available on their platforms.

There is also empathy which uses Telepathy only and already has
audio/video support with patches using farsight (which is very
active). They also have a libempathy that could be used by other
clients.
