I don't have any ideas, but CentOS 5.3 PV definitely works. Perhaps there's a problem with e1000g0 on your NIC. Are you positive there's no other client with that MAC address? Do you see any traffic on the auto-created VNIC in dom0 for that guest at all (either ingoing or outgoing) ?
