Thanks. I looked at this process and it appears to be ok, and has all the default settings from BMC.
What is also strange is that this same process works the vast majority of the time, but occasionally has this issue. In running a query, I was able to determine that this happened on 16 different change requests (out of over 400) created in the past 20 days or so, even though they seem to be virtually identical to other changes. In some cases, we even have templates that normally work (tied to product categories that bring in default approvers) that are used.
