Interesting... trixbox is running under vmware esx 3 with 2 virtual
CPUs and 256MB ram allocated.

I'm quite willing to play with the kernel to try and address this...
any good pointers as to what might be the specific issue?

The esx kernel is  2.4.21-37.0.2.ELvmnix

The CentOS kernel is 2.6.9-34.0.2.ELsmp

It can sometimes be a little tricky to find an effective combination of 
hardware and kernel version and ??? Version that all get along.  But a basic 
asterisk system as you have described does not need crazy new hardware - I ran 
very well on a celeron 350 Mhz with a similar setup.

Trixbox 2
  - FreePBX Administration 2.2.0rc3

One active trunk configured with les.net for DID and termination/origination
One grandstream budgetone 200 SIP phone

My inbound and outbound calls are all good and working perfectly...
however, when accessing the Asterisk internal recording (e.g.
voicemail, IVR, etc...) the sound quality on the line is poor with
lots of crackly and static.  I'm running with fairly close to default
settings, and have not played with different codecs yet.

The host system appears to have adequate resources -- no swapping, no
heavy load on the CPU -- and the LAN is 100-Base-T throughout.

Any ideas? Looking for some basic guidance....
