I would recommend voip.ms. Earlier this year, I needed to port my Brampton
home number to a VoIP provider that could provide E911 and could do it with
per-minute billing - voip.ms was one of the only options that fit the bill.
I have been very happy with their service, rates and the configurability of
their platform (something lacking in many providers, where you have to open
support tickets to get simple things done).

I previously used LES.NET for long distance, and continue to use them as a
backup provider to voip.ms.

I have been with unlimitel.ca for about 4 years and their service is
good;however, they are "pre-paid" serivce and you don't get money back if
you decided to leave them and you still have some money in there.

 I recently changed to freephoneline.ca for $50 (one time only); the service
(by phone) and it works very good so far. I tested that I can get two
channels in/out, but didn't test whether I can do three calls.

For a few internal call, I use voipraider.com for China. So far I violatd a
bit "fair use policy" and got charged by about us1c/min and got rest free
for the 90 days.
