It sounds like jitter. RFC2833 tends not to do well if significant
jitter is present. The latest trunk and 1.2 snapshots have some more
attempts to work around problems but really it's down to the receiving
side to deal with the jitter.

If you use "rtp debug [ip <host>]" you'll should see the outgoing
RFC2833 being clocked out at the right packet intervals (20ms normally).
Tcpdump/wireshark will confirm. If the peer sends RTCP reports and
you're using a latest build you'll also get the occasional report of
what jitter the peer sees.

If you use "sip debug" you might be able to figure out what software the
peer uses. If it's open source you could then try and add better jitter
handling to its RTP stack.

Alternatively, use SIP INFO instead of RFC2833 if the peer supports it.
That's outside the media stream, above the RTP and jitter tolerant.
