Question remains:  would anyone deploy a VM appliance to their
production architecture?  Please, please, speak up if you're willing
and under what conditions.

Its quite different situation but I'll tell that:
several polish universities are using such VM with CAS, but it's created by people we know (from one this universities) and trust them.

The main advantage is speed of deployment. Uni with no experience with Java and Tomcat can run SSO fast. Also problem solving is easier because of common environment.

Of course there is an script which simplifies firs configuration process (ex. IP, web address, certificates, etc.)

If we get a fair number of folks that answer affirmatively, then I'm
fully willing to investigate whatever technical solutions produce VMs
that folks will deploy.  On the other hand, if no one speaks up, then
it's not worth the effort, which I estimate is significant.  I spent
at least two weeks on a set of VMs that worked for _me_; productizing
a VM deployment solution would likely take months.  I'm only willing
to invest that time if there's fair interest.  I've already heard one
emphatic, "no," for security reasons.

In any case, I'm planning to put up a link to my cas-in-a-box VM set
in a couple days.  I honestly doubt anyone other than devs will find
it interesting, if that.
