On my standard '86 I have a bow plate with provision for leading (port and starboard) the anchor lines or mooring lines off the bow. The guides to a great extent prevent chafing; however, having the lines led directly forward instead of side pull on the cleats seems to have solved any loosening of the cleats.

Have you been leading your bow mooring lines through the fair lead
forward, or just tieing directly over the side off the cleats?
It makes quite a difference.  Getting into those little nuts with
washers to tighten the machine screws isn't easy, as you've probably
found out.  An allegator compression vice-clamp is a big help.
