Because the school of thought is that I should not have to pay for Billy Bob loser to get a Cat Scan. If he can't afford then he should just go and die quietly somewhere. And if he can't afford the funeral the state shouldn't pay for that either, he should just lie there and rot.
This is the system of capitalism that many Americans believe in, not any system where one's money goes toward assisting someone else that cannot pay their own way. Either you can pay your way and survive, or you cannot and then you should die. The rationalisation being there's no reason in this land of opportunity and plenty for anyone to not be able to better themselves.
