Best outcomes for the lowest cost.  How do you think your "protocols"
are created in the first place?

If you want to lower costs it seems like your big buckets would be:

(1.) Lifestyle.  Keep people healthy in the first place.  Great
example that you'll love: getting fresh vegetables in the ghetto.
Assuming someone wanted them, it's damn near impossible to find.

(2.) Making the right calls for a given set of symptoms.  This is an
art.  People who are bad artists come the system zillions.

(3.) Diagnosed people need to change their behavior.

As to premium penalties - that's how every other insurance you have
works, why should it be different with health care?
