Excuse my ignorance but don't the nvidia drivers turn Xinerama off by 
default so that TwinView works?
Looking at my working config at svn_110...

Section "ServerFlags"
    Option         "Xinerama" "0"
EndSection

My card is a Quadro NVS 280 PCI-E ,and I have the same issues...
