Hmmm.. I haven't had this situation come up but, one thing I would do would be to preface the form by using a very calm but official tone of voice and making sure it was very clear that their answers would remain confidential (?) ... and clearly give them a reason to _want_ to answer honestly. Why could it actually benefit them to answer honestly? Etc.
