Regarding ear pads for CM500, I ordered these from eBay, $13.49 including 
shipping: 

Replacement ear pads cushion earpads pad cover for KOSS UR20 UR 20 headphones

Suggested by another Elecrafter.  The ear pads fit and are just a fraction of 
an inch large, and work just fine.

It's clear these are an OEM product made for Yamaha by Koss. Koss sells 
replacement pads for their headsets $5 with free shipping. Has anyone figured 
out the correct crossover Koss model to get compatible pads?
