I can't speak about "most", because Bluebottle is the only C/R system 
I've ever used, but by default, all incoming mail which doesn't already 
match the "Approved Sender" list goes into a "Pending" folder, where it's 
held for a week, and then deleted. Part of my daily mail routine is to 
check my "Pending" folder for mail from people who haven't figured out 
how to get themselves approved (or can't be bothered), and then add the 
remaining spam to my "Blocked Sender" list, which currently numbers about 
11,000 addresses after a couple of years of use. (I put my Bluebottle 
address on my web sites in the clear, which is most of the reason that I 
get so heavily spammed, but at least my friends can reach me without 
worrying about triggering spam filter based on content.) 

There is a monthly vote for new features to add to the system, but 
apparently only about 10 other users "get" mailing lists, because there 
are only 11 votes to add the ability to add a mailing list to the 
"Approved Sender" list, so a bunch of much less important (IMHO) features 
are higher on the priority list for adding features to the system. If 
they ever do get around to doing it, then I'll be able to ditch this 
account, and run everything through Bluebottle.
