Humm... that could do it. If Spotlight is trying to index the Emailer files, then Emailer may not be able to delete the temp file until Spotlight is done with it.
This is easy to test, simply add the Emailer folder (I'd add the entire application folder) to Spotlight's Privacy settings to exclude it from being indexed. If the problem goes away, then it turned out to be that simple.
