Read the date code scratched on the EPROM at location U4. If it is not 5/2/07, then you need an update.
If it is that latest firmware, then you are having the same problem that Stuart Stevenson was having, and it had to do with some misconfiguration of the index signals between the PPMC driver and the motion control logic. Making halscope show position, index and index-enable signals is most likely going to show what is happening. Also, for safety, reduce your FOLLOWING_ERROR to 0.001 and MIN_FOLLOWING_ERROR to 0.1 inch or something, so this condition will immediately trip an E-stop rather than running away.
