Generally speaking, most of that stuff is powered down the cable, and the signal comes back on a modulated ala ntsc carrier at some handy frequency, often channel 3 or 4 in the low vhf band. The losses are pretty bad, but that is what the automatic gain control takes care of. The signal picture quality is also usually not a heck of a lot better than vhs tape.
