Make a metal holder for the nut, typically in two halves. Machine a 
block of Delrin or Acetal (they're similar but not quite chemically 
identical) to fit the holder, with a hole sized to the thread root of 
the screw it'll be running on.

Split the block in half lengthwise. Place the halves into the halves of 
the metal holder and clamp over the screw.

Heat the screw on both sides of the nut block and apply pressure until 
the plastic melts and the metal holder is pressed together. Remove heat 
and keep things still until it all cools.

Unscrew the formed nut from the screw and clean up the ends. Cut in half 
lengthwise if you're making a split nut.

You can also use oversize blocks of the plastic then with the melted 
together block still on the screw, machine it to final shape so it will 
be aligned with the screw.

Delrin and Acetal have very good lubricity, even better in graphite 
impregnated types, and will wear for a long time.
