Clearing the efreet cache fixed it for me. I think needing to clear efreet's
cache sometimes is a known issue?
My steps:
1) rm -r ~/.cache
2) killall efreetd
3) Open the Icon Theme settings, switch to another theme, Apply
4) Switch back to Adwaita.
No, this has no effect, but please note that the Thunderbird icon has not
disappeared in the main menu but only in ibar in shelves.
Oh sorry - that does sound like a different issue. Do you use a scaling
factor in Settings -> Look -> Scaling?
It sounds similar to an issue reported here:

Yes, i use scale 1.5 but no, I have the Thunderbird icon in the
Application menu but not in the ibar...
