This is a problem that has been present for awhile with a user running Jaguar X.2.4 with Office X (10.1.2 update applied). Entourage operates perfectly but anytime a calendar event or task is created with a reminder, the Office Notifications app will open and then immediately quit.
This machine does have multiple accounts on the machine but only this account uses Entourage.
Troubleshooting:
- Deleted various preference files but so far no effect.
- Ran Disk Utilities to fix permissions with no effect.

Did you try rebuilding the database?
It is possibly an early sign of mild database corruption.
Hold down the option key when starting Entourage and take the first option
('typical rebuild'). If this doesn’t resolve the problems, then try an ‘Advanced Rebuild’.
