I have no third party apps or utilities installed whatsoever (so as to specifically avoid incompatibilities like this), and have indeed experienced this bug on very rare occasion. I had just assumed that it was me screwing up, but now that I hear others experiencing it too, I feel validated that it wasn't me. >> All Entourage users probably use ⌘v and if it were an Entourage bug >> I am sure it would have attracted a lot of attention. This is the type of bug that is so easily attributable to user error that I'm not surprised it hasn't been mentioned before. I've been computing for over 25 years (so I'm no newbie), and doubt this is user error. And since I don't use any other third party apps or utilities I have to surmise that Entourage does play some part in the issue. But it's going to be difficult to find and replicate, as we only seem to experience it on rare occasion. Some rare confluence of processes is likely causing this....
