Is Outlook configured to prompt him for his credentials? If not, try doing that and explicitly entering the credential associated with his mailbox. Also remember that permission changes take up to 2 hours to propagate, so if you're making these changes and testing immediately you're not necessarily performing a valid test. Are errors being logged on the Exchange server?
