Your quote does not reflect the online terms of service.  Here are the actual TOS passages:

You agree to pay Novell ten dollars ($10 USD) for EACH E-mail sent that violates these Terms of Service and/or the SPAM policy.

The SPAM policy states that it applies to sent mail or inbound mail as a result of a mailbox used to gather responses to spam:

Sending spam E-mail from a MyRealBox address or using a MyRealBox address as a return address or an opt-out address for spam is in violation of the terms and conditions by which we operate, and users found in violation of the policy will have their account terminated. Additionally we will pursue legal actions against any person violating our terms and conditions.

So from what did you excerpt your quote?

And how does myrealbox.com stay in business?

Actually, it said $10 per email for both sending and RECEIVING spam.  Here's the excerpt.

If I violate this agreement I subject myself to legal action and will be held financially responsible for every piece of spam that goes into or out of MyRealBox.com at the amount of $10 for every piece of spam.

You obviously did not read it.

It says that you agree to pay 10$ per email IF you use your account to SPAM...

THAT is what made me think twice about (Hmmm, I really don't SPAM right? Pretty sure....yeah I am good)
