I have been through some of these on my car, particularly the "pump the brakes while on the throttle." Had a few issues early on with my car, and lost brakes a few times. Not comforting at all!

You have air in your brake lines. 2. Your brake pedal hits a mechanical stop before full pressure is applied to the brake pistons. 3. Your rotors are warped and they push the pads back. When you apply the brakes, there is too much travel in the pads to contact the rotor. A common fix for this is to pump the brakes while on the throttle. I used to do it all the time when I road raced. You don't want any surprises entering a corner at 100 MPH. 4. Your master cylinder is worn out and you can not generate enough pressure to apply the brakes properly. 5. You are low on brake fluid. 6. You did not break in your brake pads properly. See the manufactures instructions. 7. You have the wrong type of pads for your application. Call the brake pad vendor and ask what you should be using.
