This is a good practice. It leaves no ambiguity in the player's mind. Especially if there has been a dynamic change in the rest of the ensemble... then the conductor won't say "oh, everyone else is playing mf, so you should play mf too".

should a rit---always either be followed with an "a tempo" or a "q=XXX"
tempo marking?

I would. I'd use a q=XXX indication, unless you're going into a new section that has its own tempo indication. Rits, Ralls and accels need an end point. Does it slow down from q=150 to q=130 or does it go further... or not as far...?



3. (unrelated question) In a score, should rit---a tempo markings be in all
staves, or should they follow the rule for other tempo markings/rehearsal
markings? (I know they need to be in all the parts)

No. On a full score, I'd just show them like every other tempo marking.
