I finally found it. I guess I should have quadruple checked the 
machine.config file. I had added the <add name="Firebird ... element 
AFTER the </DbProviderFactories> tag. That tag came at the end of the 
previous line but everything still lined up in the editor so it looked 
OK until I happened to notice the errant tag at the end of the previous 
line. I guess I was so focused on the accurate keying of the 
PublicKeyToken and version information that I overlooked the obvious. 
Sorry for the confusion.

I hate XML.
