I've recently setup a virtual mail-hosting solution for a client using postfix + courier setup with mysql as backend. Works like a charm, squirrelmail as webmail package, postfix handles smtp, courier takes care of the pop3 and imap (webmail). It basically is the virt-mailhosting as described in the doc section from the gentoo site with some extra's tacked on. Probably will implement spamassassin + clam soon for them.
