OK I see you have considered most of the possibilities i considered.

how about: 

mount /dev/dvd /mnt/dvd
mkisofs -o mydvd.iso /mnt/dvd  

(disclaimer, other options might be needed. is mkisofs even the right
program for dvd's? k3b is making me lazy)
