Well, the referencing system itself can deal with ESRI codes,
the main likely issue I see is that, I believe, we cannot ship
with a full ESRI definitions database. I mean, everybody can take it
out of an ESRI installation, but redistributing it is probably going
to violate their licensing terms.

Right, GeoServer has been built with only the EPSG database in mind
and there are assumptions that the codes come from that database in
various parts. To locate them we'd need someone to try remove the
limitation, use ESRI codes, and see where the code breaks.
Would require some time and needs to be done for all protocols.

I have some ideas where some of the assumptions are, if you are 
interested in trying to amend the GS code drop me a note.
