It sounds like you have installed the VA Demo in Cache and have installed a potential production version in GTM. The buttons you describe probably apply to the alerts that will appear in the bottom window as you have patients with unsigned notes and abnormal test results, etc. The installation of VistA was the easy part. Now to start configuring, you need to begin to set things up. The wiki and the geocities links below are probably your best starting point.
