Been following this thread awhile, and there's a few questions I have (note
I'm on 5.09, so settings may vary):

What are two or three of the domains that are having problems?

What is the IP of the DNS server specified in the the IMail Server applet (I
think this is gone in 6.0+ - if so, what's the DNS in your control panel)?

Are you using a remote mail gateway?

What settings do you have under SMTP Security in the control panel applet?

Are you running the monitor service?

Are you logging SMTP on the debug level?

What is an IP address on the box that we can telnet to?

I'm no pro, but I've been running stable since 4.0, and I've got a little
time on my hands - help us help you.
