I bet this box has never been defragged (or at least hasn't in the 6 months I've been here). Is this maintenance I should be performing regularly on a mail server?

You would do well to use a recommended configuration which distributes functions across as many disk spindles as possible, as described in numerous places in the archives (search for "raid" and "web spool" to start). Barring other bugs--driver problems, broken hardware, disk corruption--your server will only respond as fast as your disk, and you're making one disk do logging, spooling, and mailbox duties, for example.
