Scott,
So basically you're saying that the operating system of the server itself does not need a virus shield, only the email server service?

Do most email servers have virus protection for the whole system enabled or just for the email server section?

For example McAfee has the enterprise 8.0i out that does a great job on servers and workstations in general, but McAfee also has a Command Line scanner that's been used on email servers on the spool folder.

Would it make sense to disable the enterprise 8.0i on the spool folder and run the command line scanner on the solely on this folder,
or the enterprise 8.0i could handle both task better then having them divided between the two?

That depends on what you are trying to do.

For complete E-mail protection (detecting all known viruses), you need *something* designed to protect E-mails (a mailserver virus scanner). And with that, you should not have on-access virus scanning enabled (as it will interfere with most mailserver AV products).

Note that the command line scanner by itself won't do anything -- you need to run it somehow, either manually, or using another program. If manually, you won't be able to scan incoming E-mails. If using another program, well, you need another program.
