When JFW is set to "run first" in the Jaws window, JFW puts an entry in the Run
Services key in the registry. Programs that are activated there are launched
before anything in the startup group. Incidently you can temporarily stop
programs placed in the startup group from executing if you hold down shift
whilst Windows is starting.
The main reason for the "Jaws run first" option is for those users who need
speech at network logon which happens at a fairly early stage in Windows bootup.
There may possibly be a small benefit that JFW is loaded before anything else
can impair it's performance, but I don't think that is why the facility was
added.
