We used to run our own servers at Akamai as well (Jabber back then).

So far, I've struck out in my search. It seems what was, is no more. I'm tempted to write one. If the client really wants it, I can code it. The question is how bad do they want it?

I'm not yet through looking for some existing technology though.

To be truely secure, you have to go with an openstandards product. Or roll your own. Voip over an ipsec tunnel. The Government used STUIII phones from motorola, and you had to have one at each end. If they control both ends of the conversation, then they can tunnel something. Going over the PTSN will require something along the lines of the STU though. Reading about how India doesn't like blackberry's because RIM didn't put a back door in the crytpo. Leave it to a government to want to snoop on everyone's conversation. It still cracks me up when people think "It's ok, I don't have anything to hide..."
