Well.... pretty slim notes... a name, date and church name written on a
scrap paper.  
But... you are right... that could be one way of covering it.

Your notes are a transcript of what was said. That is why if you want to

could put as privately held because you hold the notes that you took.

I had thought of using audio interview as a source -- however it seems
to be expecting a physical audio recording.  For example, it asks about
"type of medium" and the option is "privately held".  Then it asks about
the "current owner" and the "format" (for example, "audio tape") and the
year the recording is owned, etc.  So it would take a bit of twisting to
make that fit. 

For now I think I'm going to go with "Church Record" and put in the
notes that the information came from a phone conversation and that I
don't have the physical record.
