This would be the notes you took while you were on the phone with your interview. Your notes are a transcript of what was said. For example, it asks about "type of medium" and the option is "privately held". Then it asks about the "current owner" and the "format" (for example, "audio tape") and the year the recording is owned, etc. So it would take a bit of twisting to make that fit. For now I think I'm going to go with "Church Record" and put in the notes that the information came from a phone conversation and that I don't have the physical record.
