Hi Ron and Kirsten, I was having the same problems for the past few weeks, when using a source (a book) and the souce clipboard to document the pages in the book. When I attached the source to the person's name it would be attached, however if I happened to attach it a second time to the same name then I would have two duplicate sources for the persons name.
I reported this and have received the following reply: Are you using the Source Clipboard?
