Roger ! I don't know THIS modem card, but...

I think the first step (after having well read your modem manual) should
be enabling the second serial port, even if you don't use it actually.
Sure one day you'll need it and be happy not to have to change
everything all over again...
As the modem is pnp, you can try isapnptools, first to see what are your
possible modem / ComPort configurations.
Then you can use the Win95 config panel to see how it is configured
under Win95. Hopefully this corresponds to one of the possibilities
you've found with isapnptools, and is not in conflict with your other
ComPorts or other devices (the relevant items are : ComPort , IRQ , i/o
addresses, and eventually DMA, if your modem uses one). Eventually
reconfigure using isapnptools' informations. Try whether it works.
Then, back under Linux, configure your modem (eventually using
isapnptools again) EXACTLY the same way as in Win95.

Funny by nature, but here I don't laugh, I hope it works for you. This
is not my case, I've a real WinModem (should say LoseModem) :-(
