Hmm, did you try the patches I posted last week?  (*) They'll
help with the problems on cardbus powerdown or unplug, but
you didn't mention anything like that.

 From everything you've said so far -- hubs showing for the
ehci and ohci controllers on cardbus, including in that
"lsusb" output you sent -- it's behaving.  What makes you
think it's not working, the beeps?  My system beeps when
I plug in a Cardbus controller, but I thought that was
because it was telling me everything's working fine (or
perhaps just for the joy of beeping).  There's certainly
nothing it should be reporting as an error.

If your card is set up to use a DC adapter, use it ... for
some reason my CardBus adapter doesn't seem to put out enough
power over USB.  I have a lot of bus-powered devices that don't
work otherwise.  (And even some self-powered ones, which
makes me wonder about the electronics.)
