Hum... because nobody steps up to do it?

The dialog would contain a combo to select a toolbar for configuration, two listboxes "available actions" and "current actions", like most KDE apps have (cf. attached screenshot), plus a combo for "display toolbar" where you can select "always, never, context sensitive".
Session would remember these settings.

That would be wonderful. But please don't waste your time on qt3.
