I've already suggested this idea (however, I think that the ToC should be
"flat" if there is small enough).

I put it on the left side in order to leave enough room for long lines in
the menu. However, this can be changed easily.

In references, you have 'Ref: (or 'Page:') in their boxes, so they are
distinguishable from labels.
